What is FallsCan?
FallsCan is a screening tool that helps identify the risks associated with falls that can happen at any age within the home. Results of the screen can identify: the need for further assessment by a Health Professional, the need for specific adaptation or modification of the home environment, the need for exercise including balance and strength training.
It is an easy to use, web based tool that can be completed (including report and recommendations) in less than two hours.
Online Training Program
FallsCan assessors will be screened for eligibility, and then can register for the on-line training program.
Occupational Therapists, Nurses and Physiotherapists who are in good standing with a Health Regulatory College have the scope of practice to perform these assessments in the community.
The Tool
FallsCan tool provides evidence based information performed through a screening done by a health care professional. This screening will help in determining if modifications are needed, further assessment and intervention is needed or other informative suggestions which help prevent a future fall.
Tailored Reporting
Final detail reports are created, tracked for future use, emailed and saved for future editing which provides evidence to support recommendations for interventions to reduce the risk of falls to your patients clients and residents.
